Case 20 Teaching Points

  • Retinal defect involving all layers from internal limiting membrane to photoreceptor outer segments
  • Causes blurred vision and metamorphopsia: 20/60 – 20/200 range
  • May be idiopathic due to vitreal traction, traumatic, or associated with high myopia
  • Positive Watzke slit beam test
  • Rarely closes spontaneously, usually requires pars plana vitrectomy, posterior hyaloid dissection, and gas tamponade followed by face-down positioning
