Case 20 Teaching Points
- Retinal defect involving all layers from internal limiting membrane to photoreceptor outer segments
- Causes blurred vision and metamorphopsia: 20/60 20/200 range
- May be idiopathic due to vitreal traction, traumatic, or associated with high myopia
- Positive Watzke slit beam test
- Rarely closes spontaneously, usually requires pars plana vitrectomy, posterior hyaloid dissection, and gas tamponade followed by face-down positioning
