If your prescription is on file at the Kellogg Eye Center at Ann Arbor and the prescription is less than a year old, you may re-order your contact lenses by supplying the following information.

Office hours are 8:00 a.m. - 5:00 p.m., Monday through Friday.

You will be contacted by a representative from the Contact Lens Clinic to verify your order and to obtain payment information. Payment is due at the time the order is processed.
