Register before your appointment
The registration process begins once your appointment has been scheduled. This is applicable whether you are a new or returning patient. A registrar will contact you by phone prior to your scheduled appointment to complete the registration process.

As part of the process, you will be given a registration number that identifies you as a patient of the University of Michigan Health System. During the registration process, you will also be asked for the following information:  your address, phone, nearest relative, emergency contact, employer, insurance, and referring and primary care physician information. Having this information available during the registration phone call will help expedite the process. It is very important we have updated information prior to your visit.
If you are a returning patient and need to update any part of this information, please call the Registration Department. Please have your blue U-M Health System registration card available.
